Care Path Connections is a referral agency (“Agency”) that receives a fee from care services providers or supportive housing providers (collectively “Providers”) for referrals provided to or on behalf of elderly adults (“Client”) or their responsible parties by the Agency. The Agency connects Clients with options for appropriate housing and services based on each Client’s unique circumstances.
Our Process: Evaluate Clients’ needs by collecting the appropriate information including, but not limited to:
• Level of care required
• Financial resources available
• Desired geographic location
We prescreen facilities to determine the best options to meet Clients’ needs. Agency coordinates tours of the best-suited facilities (generally two to three facilities). A Placement Advisor will accompany you and provide guidance throughout the entire process. We will screen facilities for any outstanding violations and disclose to Client any deficiencies found with the Department of Social and Health Services within the last two (2) years. We may also tour Providers’ facilities prior to making a referral. Clients are not under any obligation to choose from the care options we provide. Clients are obligated to notify Care Path Connections should choose to discontinue services.
Our Fees: There is no fee for a client to use our services. There is no penalty or cancellation fee for you if you choose to discontinue use of our services. Our fees are fully paid by the providers only upon referral to the services. The fee is calculated based on the provider’s first months’ client charges. In the event the client does not reside in the provider’s care for thirty days, the fee paid by the Provider will be prorated accordingly.
Additional disclosures required by RCW 18.330: Agency may not require or request clients to sign waivers of potential liability for losses of personal property or injury, or to sign waivers of any rights of the client established in state or federal law.

The Agency works with both you and potential providers in the same transaction. As part of determining or ensuring the potential of Client using services of a Provider, the Agency may need to obtain and/or disclose the client’s confidential healthcare information.
