Care Path Grant

Care Path Connections is excited to be accepting applications for our Care Path Grant! Although we have not yet obtained all donations needed to start funding care for selected applicants we are actively working towards this goal. The Care Path Grant is designed for individuals in need of 24-hour care who cannot afford to pay for their care privately yet do not qualify for financial assistance to cover their care costs. In our years of service, we have identified a gap in the care system that we would like to actively work towards bridging. For this reason, we have created and began accepting donations to apply towards the Care Path Grant. If you believe that you or your loved one may fall into this gap, please complete the Grant Application Form below and we will reach out to you to further determine your eligibility.
